Safeguarding Forms and Assessments

There are a number of forms for used in Derby to help provide the information that is needed so that children, young people and their families receive the right services at the right time. 

Safeguarding forms

Referral to Derby or Derbyshire Children’s Social Care

Made via the online Derby Social Care Online Referral System or Derbyshire Starting Point unless the concerns are urgent.

Referral to Local Authority Designated Officer (LADO) for allegations against staff, carers and volunteers

Child in Need (CiN)

Assessment forms

There are a number of standard assessment documents to help practitioners in their work. These are kept up-to-date on the links below:

  • Early Help - Early Help Assessment
  • Domestic Abuse
    • Domestic Violence Risk Identification checklist (DVRIM) - to help assess the risk to children
    • DASH Risk Identification checklist – is used by all agencies to help identify the level of risk to victims aged 16 and above to decide whether a MARAC is needed
  • Child at Risk of Exploitation (CRE)- . Some of these tools can also be downloaded separately;
